Licensed Practical Nurse (LPN) - Full Time Evenings

Full Time Evenings, 5x8 per week. Must be able to start full-shift orientation on a Tuesday or Thursday.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents, demonstrating the knowledge and skills necessary to cater to the needs of a diverse patient population. The role entails working collaboratively with nursing staff, physicians, and ancillary personnel to ensure comprehensive patient care while maintaining a safe and healthy environment.

Responsibilities

• Collect ongoing data regarding residents' physical and psychosocial conditions, communicating findings to the Registered Nurse and Interdisciplinary Team.
• Participate in developing age-specific and culturally relevant care plans involving residents and their families.
• Evaluate resident outcomes and recommend revisions to care plans, ensuring all aspects of care are addressed.
• Maintain current knowledge and competency in nursing practice and participate in mentorship or orientation of new staff.
• Follow defined policies and procedures under the supervision of a Registered Nurse.

Required Experience / Certifications / Licensure

• Valid New York LPN Licensure.
• Basic Life Support (BLS) certification (AHA or RQI).
• Minimum of 1 year of experience as an LPN, preferably in a long-term care facility.
• Flexibility to work Day, Evening, Night, and Weekend rotations is required.
• Holiday shifts are mandatory.
