Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. Responsibilities include delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective community re-engagement, and serving as an educator to patients, families, students, and coworkers. This role adheres to the organization’s Mission Statement and Core Values while maintaining professional standards and compliance with the organizational Code of Conduct.

Responsibilities

  • Complete comprehensive evaluations of assigned patients to develop appropriate treatment plans and goals.
  • Maintain accurate and timely medical record documentation, including goal writing and charge entry.
  • Ensure care is provided under valid physician orders and within insurance approvals.
  • Participate in the student program by providing supervision and completing student evaluations.
  • Actively pursue professional development through continuing education and self-study.
  • Adhere to all regulatory agency guidelines and maintain licensure in good standing.
  • Engage in organizational performance improvement initiatives and provide feedback for quality outcomes.

Required Experience / Certifications / Licensure

  • Degree in Physical Therapy from an accredited school.
  • 1+ year of outpatient clinical experience.
  • Current CPR certification.
  • Active applicable state licensure.
  • Prior experience with Select Medical is desirable but not required.

Schedule

This position follows a 5x8.0 schedule.
