Shift: Full-Time Nights (11 PM - 7:30 AM) - 5x8s, Every Other Weekend, Holiday

Job Description

The Licensed Practical Nurse (LPN) demonstrates the knowledge and skills necessary to provide age-appropriate care to patients within their assigned unit. The LPN will assess patient data and interpret necessary information to ensure that the requirements of each patient, particularly within the adult and geriatric populations, are met.

Responsibilities

• Collect and regularly update data relevant to residents' physical and psychosocial conditions and communicate findings to the Registered Nurse (RN) and interdisciplinary team.
• Contribute to the development of age-appropriate and culturally relevant care plans reflecting nursing diagnoses and interventions aimed at achieving desired outcomes.
• Recommend revisions to care plans based on resident evaluations and facilitate necessary referrals to meet comprehensive care needs.
• Maintain knowledge and competency in nursing practices through continued education and participation in mentoring and orientation programs.
• Provide leadership under the direction of the RN, considering safety, cost, and overall impact in care delivery.
• Understand that occupational exposure to bloodborne pathogens is reasonably anticipated and be prepared for potential contact with blood, body fluids, or tissues.

Required Experience / Certifications / Licensure

• Current New York LPN License
• Basic Life Support (BLS) certification
• Minimum of 1 year experience as an LPN in a Long-Term Care Facility
