Part Time evenings, consisting of 2x12 hour shifts, every other weekend, and holidays. Must be able to begin full day orientation.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, following defined policies and procedures under the supervision of a Registered Nurse (RN). The LPN will collect data relevant to the residents' physical and psychosocial conditions and communicate findings to the RN and other members of the Interdisciplinary Team.

Responsibilities

• Collect and assess data relevant to residents' conditions on an ongoing basis.
• Contribute to the development of a culturally relevant and age-appropriate care plan.
• Recommend revisions to care plans based on evaluation of resident outcomes.
• Collaborate with residents, families, and the Interdisciplinary Team to ensure comprehensive care.
• Participate in orientation and mentoring of new staff.
• Ensure compliance with safety standards and best practices in nursing care.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification through AHA or RQI
• Minimum of 1 year of experience as an LPN, preferably in a long-term care facility
