Job Description

This position is responsible for providing diagnostic and therapeutic services, patient/family education, and appropriate referrals as necessary to patients. This will involve maintaining systematic records in accordance with professional and regulatory guidelines and ensuring quality care for all patients through interaction with hospital personnel in a coordinated, comprehensive, multidisciplinary approach.

Responsibilities

• Coordinates and directs patient care services to patients of all ages.
• Conducts patient evaluations and assessments.
• Establishes and implements personalized plans of care.
• Assesses treatment effectiveness and modifies as necessary to ensure goal achievement.
• Educates patients and caregivers regarding treatment plans and interventions.
• Produces quality documentation in a timely manner to support patient care.
• Manages billing and coding for services rendered accurately and promptly.
• Educates others using effective teaching methods.

Required Experience / Certifications / Licensure

• Bachelor’s degree in physical therapy from an accredited program; Doctorate preferred.
• Current unrestricted license as a Physical Therapist in the State of Colorado.
• BLS Certification must be obtained within 90 days of hire.
