Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. The Physical Therapist serves as an educator of patients, families, students, coworkers, and self, performing other professional duties associated with patient care and program delivery.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients towards discharge within an established length of stay.
• Document medical records clearly, concisely, accurately, and in a timely manner, inclusive of goal writing, functional quality indicator assessments, and charge entry.
• Ensure care is provided under signed, valid physician orders and within insurance approval as necessary.
• Participate in student programs as assigned, providing a positive learning environment for students.
• Create an individualized professional development plan and pursue continuing education and self-study.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives and follow through with action plans.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active state licensure as a Physical Therapist in good standing.

Schedule

This position offers a 5x8 schedule.
