LPN - Full Time - Nights

5x8 hours per week, with every other weekend and holiday required.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents, ensuring that care is appropriate to their age and developmental needs. The LPN will collaborate with Registered Nurses (RNs), physicians, and ancillary staff, following established policies and procedures to promote health and safety within the care environment.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans in collaboration with residents and their families.
• Evaluate resident outcomes, recommending revisions to care plans as necessary.
• Assist in mentoring and orienting new staff to ensure a competent nursing team.
• Maintain professionalism, demonstrating knowledge and clinical expertise through continuous education.
• Follow safety protocols, considering factors related to cost and environmental impact in care delivery.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (American Heart Association or RQI)
• At least 1 year of experience as an LPN, preferably in a Long Term Care (LTC) facility.
