Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with their needs, goals, and physician direction. Responsible for directing all discipline-specific therapy services, the therapist ensures the design of functional treatment programs for effective community reintegration. This role also includes educating patients, families, students, and coworkers, while upholding professional standards and maintaining compliance with the organization's Mission Statement and Core Values.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop effective treatment plans aimed at discharge within established length of stay.
• Document medical records clearly and concisely in compliance with state, federal, and organizational guidelines.
• Provide care in accordance with signed, valid physician orders and insurance approvals.
• Participate in the student program, fostering a positive learning environment with appropriate supervision.
• Create a personalized professional development plan in collaboration with clinical management.
• Adhere to state practice acts and maintain professional licensure while completing required continuing education.
• Engage in performance improvement initiatives to maximize quality of services.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license where work will be performed.

Schedule

This position follows a 5x8.0 schedule, requiring 3 days at Royal Palm Beach and 2 days at Wycliffe.
