Part-Time Licensed Practical Nurse (LPN)

Schedule

Part-time Day and Evening shifts, scheduled as needed by the facility (generally 2x8s/3x8s). Weekend and holiday shifts are required.

Job Description

The Licensed Practical Nurse (LPN) provides direct and indirect skilled nursing care to assigned residents and demonstrates the knowledge and skills necessary to provide age-appropriate care for adult and geriatric patients. Under the supervision of a Registered Nurse, the LPN will follow defined policies and procedures, collaborate with other healthcare professionals, and maintain clinical expertise through ongoing education.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis, communicating findings to the RN and members of the interdisciplinary team.
• Contribute to the creation of individualized, culturally relevant care plans based on nursing diagnoses and interventions.
• Recommend revisions to care plans as needed and assure thorough communication with residents, families, and team members to promote satisfaction and optimal care.
• Maintain up-to-date knowledge and competency in nursing practices, participating in mentoring and orientation of new staff.
• Plan and deliver safe, effective care considering various factors including safety and cost.
• Assume leadership responsibilities in the professional practice setting while ensuring compliance with health and safety regulations, particularly in relation to blood-borne pathogens.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS certification through AHA or RQI.
• Minimum of 1 year experience serving in an LPN capacity at a long-term care facility.
