Job Description

The Registered Nurse (RN) in Home Health is responsible for providing skilled nursing care to patients in their homes, with a focus on rehabilitation, disease management, and education for patients and their families. The position requires home health experience and proficiency in OASIS documentation.

Responsibilities

• Conduct patient assessments and evaluations
• Perform skilled nursing interventions and treatments
• Educate patients and families on disease management and home exercise programs
• Document patient care and interventions accurately in compliance with OASIS standards
• Manage medication administration and monitoring
• Utilize adaptive devices and equipment for patient mobility and safety
• Provide education on safety and injury prevention
• Identify signs and symptoms of physical, sexual, and psychological abuse

Required Experience / Certifications / Licensure

• Minimum of 1 year of nursing experience in Home Health
• BLS certification required
• Valid Florida RN license or compact license
• OASIS training preferred
• Must be COVID vaccinated or have an approved exemption
• Flu shot or declination required
• Must possess a valid driver's license and have a reliable vehicle for home visits
