Outpatient Physical Therapist - 13-Week Assignment

Job Description

The Outpatient Physical Therapist evaluates assigned patients, developing, implementing, and modifying treatment plans according to their needs, goals, and physician direction. Responsibilities include delegating and directing all discipline-specific therapy services while ensuring the design of functional treatment programs that facilitate an effective return to community activities. The therapist serves as an educator for patients, families, students, and coworkers, while adhering to organizational standards of compliance, including maintaining confidentiality and upholding professional codes of conduct.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Complete all necessary medical record documentation promptly and accurately, including goal writing and functional quality indicator assessments.
• Ensure care is provided under valid physician orders and align with insurance approvals.
• Participate in the supervision and mentorship of students during their clinical programs.
• Engage in continuous professional development through education and involvement in professional organizations.
• Follow the organization’s performance improvement initiatives to enhance the quality of services provided.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Maintaining professional licensure in good standing, including completion of all state-mandated continuing education requirements.

Schedule

This is a 5x8.0 shift position.
