Job Description

The Case Management Department is seeking a dedicated Case Manager to join our team. The ideal candidate will have prior experience in case management, particularly within an HCA facility. The position requires the ability to manage patient care effectively, ensuring optimal outcomes while coordinating between various departments.

Responsibilities

• Manage patient care coordination and case management services.
• Collaborate with healthcare professionals to optimize patient outcomes.
• Utilize Meditech and Midas programs to document patient information.
• Maintain compliance with documentation requirements and policies.
• Participate in multidisciplinary team meetings.

Required Experience / Certifications / Licensure

• Prior experience in case management at an HCA facility is required.
• Experience with Meditech and Midas programs is preferred.
• Relevant certifications will be considered an asset.
