08:00 AM - 05:00 PM, with every other weekend and holiday rotation.

Job Description

We are seeking an experienced Registered Nurse specializing in Case Management. The role will require collaboration with a team of healthcare professionals to support patient care and ensure effective treatment plans are followed.

Responsibilities

• Manage and coordinate patient care in an acute care setting.
• Conduct comprehensive case assessments and evaluations.
• Develop and implement effective care plans.
• Collaborate with multidisciplinary teams, including Social Workers and SNF coordinators.
• Provide support and advocacy for patients and their families.

Required Experience / Certifications / Licensure

• Current RN license in the state of Florida.
• Experience in Case Management, particularly in acute care settings.
• Familiarity with Meditech and Midas is preferred.
