Part-Time Nights (2x8s/3x8s), every other weekend. Must be able to begin orientation on a Tuesday or Thursday.

Job Description

Under the direction of licensed nursing personnel, the LPN performs direct and indirect designated resident care within the scope of practice as defined by certification. The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served, focusing on the adult and geriatric patient groups.

Responsibilities

• Ensure residents' Activities of Daily Living (ADLs) needs are met as outlined in the plan of care.
• Maintain residents' environment and safety in accordance with policies and procedures.
• Meet residents' nutritional needs, honoring preferences and choices.
• Address residents' rehabilitative needs per the plan of care.
• Report residents' rights, refusals, and changes in physical, emotional, and psychosocial status timely.
• Adhere to occupational health and safety protocols, particularly regarding blood borne pathogens.

Required Experience / Certifications / Licensure

• New York LPN Licensure.
• BLS certification (AHA or RQI specifically).
• Minimum of 1 year experience as an LPN.
• High School Diploma or GED.
