Schedule

Day shift, 5x8-hour schedule (08:00 - 16:30). This facility cannot accommodate alternative schedules such as 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Physical Therapist Assistant to join our team. The successful candidate will be responsible for providing 1:1 patient care, visiting up to 6 patients per day, with a productivity standard of 30 points.

Responsibilities

• Provide skilled home health physical therapy services to patients in their homes.
• Conduct assessments and develop treatment plans in collaboration with healthcare teams.
• Monitor patients' progress and adjust interventions as necessary.
• Document patient care and maintain accurate records using EPIC Remote Client for Home Health and Rover.

Required Experience / Certifications / Licensure

• 1 year of skilled home health experience required.
• Current BCLS certification.
• Active licensure in Oregon or Washington is required.
