Job Description

We are seeking a highly qualified RN Case Manager to coordinate and manage care while ensuring optimum utilization of resources, service delivery, and compliance with external review agencies. The ideal candidate will provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. This role is essential in enhancing the quality of patient management and satisfaction, promoting continuity of care, and ensuring cost-effectiveness through the integration of case management functions.

Responsibilities

  • Coordinate management of care and ensure compliance with external review agencies.
  • Conduct comprehensive assessments and develop care plans tailored to individual patient needs.
  • Implement care plans and evaluate the effectiveness of care delivered.
  • Support discharge planning and resource utilization to advocate for patients’ holistic needs.
  • Work collaboratively with the interdisciplinary team to promote optimal patient outcomes.

Required Experience / Certifications / Licensure

  • Must have a minimum of 2 years recent RN Case Management experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
  • Experience with MCG guidelines and discharge planning across acute care settings is required.
  • Strong collaboration skills with the interdisciplinary team are essential.
  • Current California RN license is required.
  • BLS certification through the American Heart Association must be provided.

Schedule

This position offers a Monday to Friday schedule from 8:00 AM to 4:30 PM.
