Job Description
The Case Manager at the Acute Rehabilitation Unit will be responsible for managing patient care plans, assessing patient needs, and coordinating services effectively across the continuum of care.
Responsibilities
- Conduct comprehensive assessments of patient conditions and needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Monitor patient progress and adjust care plans as necessary.
- Coordinate care transitions between different units and settings, including opportunities to float to LTACH if open.
- Documentation of care coordination and patient interactions in the EMR system.
Required Experience
- Experience in case management.
- Previous experience in hospital settings is required; ARU experience is preferred but not necessary.
Certifications/Licensure
- Licensed Social Worker (LSW) in the state of Florida.
