Job Description

The Physical Therapist will provide skilled home health visits, focusing on a diverse patient population including those recovering from surgical procedures, individuals with cardiopulmonary issues, and various musculoskeletal disorders. Responsibilities include conducting patient assessments, developing home exercise programs, and ensuring safety during patient mobility using specialized equipment. The therapist will document patient progress and coordinate care with family members and other healthcare providers.

Responsibilities

• Conduct thorough patient assessments and evaluations.
• Develop and implement individualized home exercise programs.
• Utilize equipment such as gait belts, walkers, and lifts to assist patient mobility.
• Document all patient interactions and progress accurately in the BayCare documentation system.
• Educate patients and families on care plans and rehabilitation processes.
• Ensure patient safety and adhere to safety protocols while in the home health environment.

Required Experience

• Minimum of 1 year of home health experience is required.
• OASIS training preferred.

Certifications/Licensure

• Current state licensure as a Physical Therapist.
• Basic Life Support (BLS) certification is required.
