Day Shift: 8:00 AM - 4:30 PM, with one weekend per month required.

Job Description

As a Registered Nurse in Home Health, you will provide in-home clinical care for patients, ensuring a high standard of services. This role involves a significant amount of documentation and you will be expected to manage a high volume of patients each day. Experience with OASIS is a key requirement for this position.

Responsibilities

• Deliver home health nursing services to patients in accordance with established health care plans.
• Perform nursing assessments and provide education to patients and their families.
• Complete documentation and reports as per regulatory guidelines.
• Manage a caseload of 5-6 patients per day, focusing on quality care and compliance.
• Participate in interdisciplinary team meetings and contribute to care planning.

Required Experience

Minimum of 2 years of home health experience required. Strong knowledge of OASIS documentation is essential.

Certifications/Licensure

Valid New Jersey RN license required.
