Job Description

As a Primary Registered Nurse at HopeWest, you will embody the compassionate care that patients and families seek, making a significant difference in their lives. You will engage with patients to identify their personal goals of care and provide support to their caregivers, ensuring they feel confident in managing care for their loved ones. Your role requires a strong set of clinical skills, a broad knowledge of various diseases, and expertise in symptom management.

As the leader of the interdisciplinary team for your assigned caseload, you will provide care in various settings, including private homes, nursing homes, group homes, and assisted living facilities. Responsibilities

• Provide compassionate hospice care to patients in various settings.

Required Experience

Strong clinical skills in hospice care and experience in providing patient-centered care are required.

Certifications / Licensure

A valid Registered Nurse license is required to practice in the state of Colorado.
