Schedule

Shift: 8:00 AM - 5:00 PM, with on-call responsibilities. This position includes both scheduled visits and on-call responsibilities throughout the shift.

Job Description

The Home Health Registered Nurse provides full-time nursing services to patients in their homes, under the supervision of the Clinical Services Supervisor, in accordance with agency standards and policies.

Responsibilities

• Initiates and provides medically prescribed skilled nursing care and health counseling to contribute to patient treatment and symptom management.
• Conducts initial visits to assess the patient's physical and emotional status, needs, and resources.
• Develops a nursing plan of care based on physician's treatment plan and identified problems from the nursing assessment.
• Educates patients using the Care Plan/Standard of Care.

Required Experience / Certifications / Licensure

• 2+ years of hospice experience (REQUIRED)
• Travel capability (PREFERRED)
• Home Care Home Base experience (REQUIRED)
• OASIS experience (PREFERRED)
• BLS Certification (AHA)
• Current Michigan State RN License
