Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective returns to community activities. Additionally, the Physical Therapist serves as an educator for patients, families, students, and coworkers.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Complete medical record documentation in a clear, concise, accurate, and timely manner in compliance with all relevant guidelines.
• Ensure care is provided under signed, valid physician orders and within insurance approval as necessary.
• Participate in student program supervision and evaluations as appropriate.
• Create individualized professional development plans and pursue ongoing education and involvement in professional activities.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• A degree in Physical Therapy from an accredited school.
• Minimum of 2 years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure.
• Previous experience with the agency is highly desired.

Schedule

This position involves a 5x8.0 schedule, requiring flexibility with a demand for 40 hours including two evenings per week.
