Job Description

The Physical Therapist will be responsible for providing comprehensive assessments and developing tailored care plans to enhance patient functionality and independence. This role requires close collaboration with clinical team members and caregivers, as well as the supervision of support staff, to ensure optimal patient care.

Responsibilities

• Monitor and adjust patient visit schedules to optimize productivity and meet patient needs.
• Conduct thorough OASIS and physical therapy assessments, documenting findings in accordance with regulations.
• Create individualized plans of care with specific goals and interventions to enhance patient outcomes.
• Provide skilled physical therapy treatment and assess patient progress on each visit.
• Coordinate patient care with a multidisciplinary team including physicians, nurses, and other healthcare providers.
• Supervise and direct the clinical activities of PTAs, students, and home care aides.
• Complete necessary documentation for clinical activities and insurance billing.
• Participate in training programs and community outreach to promote rehabilitation services.
• Perform various patient assessments and procedures as required.
• Ensure timely communication regarding patient care schedules and issues.
• Foster safe patient discharges and manage patient incident reports as needed.
• Engage in peer reviews and collaborative quality improvement initiatives.

Required Experience / Certifications / Licensure

Active Physical Therapist licensure required. Experience in home health or rehabilitation settings preferred.
