Job Description

The Licensed Practical Nurse (LPN) will be an integral part of the Transitions of Care program, which aims to reduce 30-day readmission rates. This mobile position involves travel to patients' homes, hospitals, and provider offices to support care coordination under the supervision of an RN Care Manager.

Responsibilities

• Assist with implementing care plans in collaboration with RN Care Managers.
• Schedule appointments and monitor biometric data.
• Provide and reinforce education to patients and families regarding care.
• Attend patient visits as necessary and complete home visits.
• Connect patients with community resources and maintain regular patient contact.
• Document patient interactions and updates in the electronic health record (EHR).
• Participate in care team rounds to provide updates on patient status.
• Manage a caseload of approximately 25 to 30 patients monthly.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in a relevant field.
• Current BLS certification is required.
• New York State LPN license is required.
• Knowledge or experience with chronic conditions such as CHF and COPD is preferred.
• Experience with EHR systems, specifically Epic, is a hard requirement.
