Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Transitions of Care program. This mobile position requires travel, including potential home visits, hospital visits, and patient office visits. The LPN will work under the direction of the RN Care Manager to implement care plans that aim to reduce readmission rates.

Responsibilities

• Assist with the implementation of care plans for clients under the direction of the RN Care Manager.
• Schedule appointments and monitor biometric data.
• Provide and reinforce education to patients regarding their health conditions.
• Attend patient visits and complete home visits as necessary.
• Connect patients with community resources to support their care.
• Maintain regular contact with patients and document all interactions in the EHR.
• Participate in care team rounds to provide updates on patient progress.
• Manage a monthly rolling caseload of approximately 25-30 patients.

Required Experience / Certifications / Licensure

• Current LPN license in New York State.
• Minimum of 1 year of experience in a related field, preferably in Home Health or Mobile Case Management.
• BLS certification is required.
• Preferred experience with chronic conditions such as CHF, COPD, and complex comorbidities.
• Familiarity with care coordination and community resource knowledge is a plus.
