Job Description

We are seeking an experienced Licensed Practical Nurse (LPN) for a Mobile Care Manager position to support our Transitions of Care program. This role focuses on reducing 30-day readmission rates by providing comprehensive care management to adults and geriatric patients in various settings, including home visits, hospital visits, and office visits. The LPN will work closely with the RN Care Manager, assisting in implementing care plans without performing hands-on nursing tasks.

Responsibilities

• Appointment scheduling and biometric data monitoring.
• Providing and reinforcing patient education under the supervision of an RN.
• Completing home visits and connecting patients with community resources.
• Maintaining frequent patient contact and managing a caseload of approximately 25-30 patients.
• Documenting patient interactions and care plans in the Electronic Health Record (EHR).
• Participating in care team rounds to provide patient updates.

Required Experience / Certifications / Licensure

• Minimum of 1 year of relevant experience in home health or mobile case management.
• BLS certification is required.
• Active New York LPN license required.
• Experience with patients diagnosed with CHF, COPD, and multiple complex comorbidities is preferred.
• Familiarity with case management and community resources is strongly preferred.
