Registered Nurse - Home Care

Job Description

The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive medical care to help older individuals continue living in the community. The Clinical Care Registered Nurse (CCRN) will offer direct nursing care to participants and will manage complex clinical situations while collaborating with an interdisciplinary care team.

Responsibilities

• Triaging emergent situations while applying advanced nursing knowledge.
• Monitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
• Utilizing care plans for clinic and home-based direct care.
• Administering treatments, procedures, and medications as prescribed.
• Performing routine monitoring of participants' medical conditions.
• Providing wound care and treatments as specified in care plans.
• Conducting regular home visits for care, treatment, and assessments.
• Addressing on-call needs and determining the immediate requirements for participants.
• Documenting participant care in accordance with policy.
• Collaborating with staff and families in a participant-centered care model.

Required Experience / Certifications / Licensure

• Bachelor of Science in Nursing preferred; Associate degree required.
• Current RN Nursing Diploma or Degree from an accredited institution.
• A minimum of one year of experience with the elderly or long-term care population.
• Home care experience and wound care experience preferred.
• Current CPR certification or willingness to obtain.
• Reliable transportation with a valid Michigan driver’s license and current vehicle insurance.
• Up-to-date immunizations and vaccines or medical documentation for waivers.

Schedule

Monday - Friday, 8:00 am - 4:30 pm

Assignment Duration

This is a 13-week contract position.
