Job Description

We are seeking a skilled Licensed Practical Nurse (LPN) for a Mobile Case Management position focusing on adult and geriatric patients. This role requires travel for home visits, hospital visits, and office visits, contributing to a multidisciplinary care team aimed at reducing readmission rates.

Responsibilities

• Support the Transitions of Care program by implementing care plans under the direction of the RN Care Manager.
• Conduct appointment scheduling and biometric data monitoring.
• Educate and reinforce health management practices with patients.
• Complete home visits and attend patient visits as needed.
• Connect patients with community resources and maintain frequent contact.
• Document patient interactions and updates in the electronic health record (EHR).
• Participate in care team rounds to provide updates and insights on patient status.

Required Experience / Certifications / Licensure

• 1 year of experience as an LPN in a home health or case management setting.
• Basic Life Support (BLS) certification required.
• Proficiency in using the Epic charting system.
• NY State LPN license is required prior to submission.
• Experience with chronic conditions such as CHF and COPD is preferred.
