LPN - Transitions of Care Program

Job Description

We are seeking a dedicated LPN for a full-time mobile position focusing on the Transitions of Care program. The ideal candidate will support efforts in reducing 30-day readmission rates, working under the direction of the RN Care Manager. This is not a hands-on nursing role, but rather a position that emphasizes care coordination and patient education.

Responsibilities

• Appointment scheduling and biometric data monitoring.
• Providing and reinforcing patient education.
• Attending patient visits as needed and completing home visits.
• Connecting patients with community resources and maintaining frequent contact.
• Documenting in EHR and participating in care team rounds.
• Managing a monthly rolling caseload of approximately 25-30 patients.

Required Experience / Certifications / Licensure

• Minimum of 1 year of nursing experience, preferably in home health or case management.
• BLS certification required.
• Active LPN license in New York is required.
• Experience with chronic conditions such as CHF and COPD is preferred.
• Proficiency in EHR, specifically Epic, is mandatory.
• Access to reliable transportation for required travel during work hours.
