Registered Nurse - Case Management

Job Description

The Registered Nurse will perform case management and coordinate the discharge planning care process for age-appropriate in-patients with chronic diseases and special health care needs. This role will be based within the Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit, and involves maintaining a tracking system for referred patients for admission with other healthcare facilities while performing chart reviews for requested services related to return to facility or discharge home.

Schedule

This is an 8-hour shift position, Monday through Friday, from 8:00 a.m. to 5:00 p.m. Travelers are required to attend staff meetings when scheduled.

Required Experience / Certifications / Licensure

• Two (2) years or more of experience in discharge planning and medical-surgical (Med-Surg) experience.
• Must have RN experience in a clinical, medical/surgical, acute, and/or critical care setting.
• Valid and unrestricted Registered Nurse license in any U.S. State or Territory.
• Valid American Heart Association (AHA) certification in Basic Life Support (BLS), which must be maintained.
