Job Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our rehabilitation team. The ideal candidate will have acute experience and be part of an inpatient rehabilitation setting focused on maximizing the health, function, and quality of life of individuals recovering from severe injuries or illnesses.

Responsibilities

• Provide direct patient care under the supervision of registered nurses.
• Assist patients with activities of daily living (ADLs).
• Monitor and document patient status and report changes to the healthcare team.
• Administer medications and treatment protocols as prescribed.
• Assist in developing individualized care plans.
• Work collaboratively with the rehabilitation team to promote recovery.

Required Experience / Certifications / Licensure

• Valid LPN license.
• Current BLS certification.
• Minimum of 2 years of acute care experience required.
