Job Description

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) with acute experience to join our rehabilitation team. The LPN will play a crucial role in providing comprehensive care to patients recovering from severe injuries or illnesses such as stroke, brain injury, and amputations. This position involves working closely with a diverse patient population to enhance their recovery journey through specialized rehabilitation programs.

Responsibilities

• Provide direct patient care and assist with daily activities.
• Monitor and document patient progress and response to treatments.
• Administer medications and perform medical procedures as required.
• Collaborate with multidisciplinary teams to develop and implement patient care plans.
• Educate patients and their families on health management and recovery strategies.

Required Experience / Certifications / Licensure

• Current LPN license
• BLS certification
• Minimum of 2 years of experience in an acute care or rehabilitation setting
• Associate or Bachelor’s degree in Nursing
