Licensed Practical Nurse (LPN) - 8HR Rotating Shift

Job Description

The Licensed Practical Nurse (LPN) plays a vital role as a member of the healthcare team, utilizing sound nursing judgment, preparation, knowledge, and experience in providing care in accordance with UPMC LPN Practice Guidelines and State Board Regulations. The LPN collaborates with Registered Nurses and other licensed providers in various healthcare settings to effectively implement and evaluate patient care.

Responsibilities

• Utilize sound nursing judgment to assess and manage patient care.
• Participate in planning, implementation, and evaluation of nursing care using focused assessments.
• Communicate effectively with health care team members regarding patient care needs and significant changes in patient conditions.
• Follow established policies and procedures while providing care and obtaining supervision when necessary.
• Formulate daily goals and individualized care plans in partnership with patients.
• Document patient information comprehensively to enhance communication between caregivers.
• Provide compassionate and effective patient and family education throughout the care process.
• Collaborate with the healthcare team to achieve desired patient outcomes and maintain productive working relationships.
• Identify and communicate any safety hazards within the workplace.

Required Experience / Certifications / Licensure

• Successful completion of a Licensed Practical Nursing Program approved by the State Board of Nursing.
• 2 years of experience is required.
• Current Basic Life Support (BLS) certification.
• Active Licensed Practical Nurse (LPN) license in Pennsylvania.
• Completion of PA Acts 31, 33, 34, and 73.

Schedule

This position requires a rotating shift schedule of 8 hours over 5 days a week.
