Friday, Saturday, Sunday, and Monday from 8 am to 6 pm.

Job Description

We are seeking a skilled Home Health Nurse with OASIS experience to join our team. The successful candidate will manage patient care in a home health setting, offering compassionate and comprehensive services to enhance patient recovery and quality of life. The role requires flexibility and adaptability to changing schedules and patient needs.

Responsibilities

• Conduct OASIS assessments and implement care plans for home health patients.
• Travel to patients' homes to provide nursing care and support.
• Coordinate with interdisciplinary teams to ensure comprehensive patient care.
• Document patient progress and adjust care plans as necessary.
• Respond to emergencies and provide support during scheduled on-call shifts.

Required Experience / Certifications

• Minimum of 2 years of Home Health and Hospice experience.
• Proven experience with OASIS documentation.
• Must possess a valid driver's license in good standing.
• Current Basic Life Support (BLS) certification is required.
• Familiarity with EMR systems, specifically HealthWyse (Careficient), is preferred.
