Part Time RN - Licensed Practical Nurse

Schedule

This is a part-time position with variable day or night shifts as needed. Weekend and holiday work are required.

Job Description

The Registered Nurse (RN) will provide comprehensive care to residents, emphasizing age-appropriate and culturally relevant care for both adult and geriatric populations. The RN will collaborate with an interdisciplinary team to assess, plan, implement, and evaluate patient care strategies.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to supervisors and team members.
• Contribute to developing care plans that reflect nursing diagnoses and desired outcomes, involving residents, families, and the healthcare team.
• Recommend revisions to care plans based on resident evaluations, ensuring all aspects of care are addressed.
• Maintain current knowledge and competency in nursing practice and participate in mentoring and orientation of staff.
• Demonstrate leadership in the professional practice setting under the direction of the Nurse Manager.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure.
• BLS certification (AHA or RQI only).
• At least 1 year of experience as an RN with a focus on long-term care (LTC).
