Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for the assigned patients while ensuring the design of functional treatment programs that allow for an effective return to community activities. The therapist serves as an educator of patients, families, students, coworkers, and self, performing other professional duties associated with patient care and the development of discipline-specific therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients towards discharge.
• Document care clearly, concisely, and accurately following state, federal, and clinic guidelines.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in the student program as assigned, providing a positive learning environment for students.
• Create an individualized professional development plan and pursue ongoing professional growth.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and implement organizational performance improvement methodologies.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR Certification.

Schedule

This is a 5x8.0-812000 shift position starting ASAP.
