Job Description

CVHCare is seeking a qualified Physical Therapist to deliver exceptional patient care in designated territories. The position requires effective communication and collaboration with patients and healthcare providers.

Responsibilities

• Conduct evaluations and create treatment plans for patients.
• Provide routine therapy sessions and assessments.
• Document patient progress and adjust treatment plans as necessary.
• Utilize OASIS assessments where applicable.
• Maintain productivity standards and caseload expectations.

Required Experience / Certifications / Licensure

• Current California Physical Therapist License
• BLS certification
• Valid Driver's License and auto insurance
• TB screening (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual)
• Drug screen and physical examination
