Job Description

We are seeking a dedicated Acute Care Physical Therapist to provide high-quality therapy services in a community-based acute care hospital setting. The ideal candidate will assess, plan, and implement treatment plans to aid in rehabilitating patients with various diagnoses, including postoperative care, geriatrics, and general medical conditions.

Responsibilities

• Conduct evaluations and develop treatment plans for patients.
• Implement intervention strategies focusing on mobility, strength, and functional recovery.
• Educate patients and their families on home exercise programs and rehabilitation goals.
• Monitor and document patient progress and adjust plans of care as necessary.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum 1 year of experience in an acute care setting.
• Current California Physical Therapist license.
• Basic Life Support (BLS) certification.
• Experience with Epic charting system is preferred.
