This is a 13-week contract position.

Schedule

Start ASAP through September 11. The schedule is Monday, Tuesday, and Thursday from 8 AM to 7 PM; Wednesday and Friday from 8 AM to 1 PM.

Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans based on patient needs, goals, and physician direction. This role includes delegating and directing therapy services, educating patients and families, and ensuring functional treatment programs for successful community reintegration. Adherence to the organization’s Code of Conduct and core values is essential, fostering a safe clinical environment for both patients and staff.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and create individualized treatment plans.
• Document patient records clearly, concisely, and accurately, ensuring compliance with all guidelines.
• Ensure care is delivered under valid physician orders and within insurance approvals.
• Participate in educational programs for students and colleagues.
• Engage in ongoing professional development to enhance clinical knowledge and skills.
• Comply with state practice acts and maintain licensure in good standing.
• Contribute to performance improvement initiatives within the organization.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for the role.
• Previous experience with Select Medical is desirable.
