Job Description

We are seeking a skilled Home Health Registered Nurse to provide patient care in the comfort of their homes. The ideal candidate will demonstrate proficiency in executing OASIS assessments, starting care, and conducting revisits.

Responsibilities

• Conduct comprehensive patient assessments and develop care plans based on OASIS standards.
• Manage and coordinate patient care in collaboration with healthcare teams.
• Perform regular visits to evaluate patient progress and modify plans as necessary.
• Document patient care activities in the electronic health record (EPIC).

Required Experience / Certifications / Licensure

• Recent home health experience is required.
• EPIC experience is required.
• OASIS certification is required.
• A valid RN license is necessary.
