Job Description

We are seeking a skilled Case Manager to join our dynamic team. The ideal candidate will have recent acute care discharge planning experience and will be responsible for managing patient care coordination and discharge planning processes in a 590-bed hospital environment.

Responsibilities

• Coordinate and manage discharge planning for patients in acute care.
• Collaborate with multidisciplinary teams including Nursing, Social Work, and Clinical Medical Assistants.
• Utilize documentation systems such as Meditech, Midas, and Wellsky effectively.
• Ensure compliance with hospital policies and patient care standards.

Required Experience / Certifications / Licensure

• Must have recent acute care discharge planning experience.
• Relevant certifications in case management are preferred.
• Professional demeanor with ability to dress in business casual attire.
