AM 5 × 8 hr shifts, Monday-Friday, 40 hours per week.

Job Description

We are seeking a skilled Home Health Physical Therapist to provide quality care in a home health setting. The ideal candidate will be responsible for delivering physical therapy services to patients in their homes, conducting evaluations, and developing treatment plans tailored to patient needs.

Responsibilities

• Conduct initial evaluations and assessments of patients.
• Develop and implement individualized treatment plans.
• Provide ongoing physical therapy interventions and documentation.
• Collaborate with healthcare team members to ensure comprehensive care.
• Maintain accurate and timely documentation in HCHB.

Required Experience / Certifications / Licensure

• Active Physical Therapist (PT) license.
• Home Health and OASIS experience required.
• Experience with Homecare Homebase documentation preferred.
• Ability to travel within a 60-mile radius from the facility address.
