Physical Therapist (PT)

Job Description

Fall River Health Services is seeking a dedicated Physical Therapist (PT) to provide high-quality treatment for individuals experiencing illness or injury. The primary focus of this role is to rehabilitate and restore bodily functions using specialized physical exercises and therapies, promoting overall wellness and patient independence. The PT will be responsible for developing therapy programs tailored to patients of all ages, addressing developmental deficits, physical injuries, or illnesses.

Responsibilities

• Coordinate Physical Therapy (PT) services and deliver direct therapeutic care to assigned clients.
• Teach rehabilitation skills to clients and caregivers that enhance functional independence and safety.
• Provide preventive and rehabilitative PT procedures tailored to individual client needs.
• Develop and revise plans of care based on continual reassessment, client history, diagnosis, and physician orders.
• Perform or oversee treatment applications involving physical agents such as heat, water, and electricity.
• Create and implement teaching plans, and communicate significant clinical findings to relevant personnel.
• Document all care thoroughly, ensuring compliance with Medicare and other local policy requirements.
• Maintain organizational confidentiality and participate in Quality Management programs.
• Participate in staff meetings and contribute to business development initiatives.
• Deliver outstanding customer service to both internal and external customers.
• Supervise Physical Therapy Assistants and Home Health Aides as applicable.

Schedule

This position involves 8-hour shifts, from Tuesday to Saturday.
