Days: 8am-5:30pm (Schedule can be flexible based on patient's availability.)

Job Description

The Physical Therapist's primary goal is to restore function to prevent or minimize disability following disease or injury, as well as to help the client reach his/her maximum performance/function within the limits of his/her capabilities. The Physical Therapist will evaluate and treat patients using medically prescribed physical therapy programs. Responsibilities include supervising Physical Therapy Assistants, making initial admission/evaluation visits, and notifying the assigned Clinical Team Manager of OASIS scores within established timelines.

Responsibilities

• Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.
• Evaluate and treat patients according to established physical therapy programs.
• Supervise Physical Therapy Assistants.
• Make initial admission/evaluation visits.
• Notify the Clinical Team Manager of OASIS scores within timelines.
• Perform other duties as assigned consistent with skills and training for this position.

Required Experience / Certifications / Licensure

One year of Home Health experience required.
