Occupational Therapist - Home Health

Job Description

CVHCare is seeking a qualified Occupational Therapist for a home health role. This position welcomes new graduates and offers flexible scheduling options, including guaranteed and non-guaranteed hours. The territory includes areas such as Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, and options for Oakland, Berkeley, or San Ramon areas.

Responsibilities

• Conduct patient assessments, evaluations, and develop effective treatment plans.
• Maintain documentation of patient care and ensure compliance with standards.
• Collaborate with healthcare team members to deliver comprehensive patient care.
• Travel to designated patient locations within assigned counties.
• Be willing to see patients who are Covid+ or persons under investigation for Covid.

Required Experience / Certifications / Licensure

• Current Occupational Therapist (OT) license in California.
• Basic Life Support (BLS) certification.
• Valid driver's license and auto insurance.
• Annual Tuberculosis (TB) screening, MMR, Varicella, Hepatitis B, Tdap (within last 10 years), Flu (annual), Drug screening, and Physical examination.

Schedule

This is a day shift position, with potential weekend coverage required. The workload may require traveling outside the assigned service area to maintain a full caseload.
