Physical Therapist - Rehabilitation Services

Days (8:00 AM - 4:30 PM), no weekends

Job Description

The Rehabilitation teams evaluate and treat patients with injuries or conditions that have impacted their physical mobility and functional independence. Our specialized services include orthopedic rehab, neurological and stroke rehab, balance and dizziness rehab, and chronic pain treatment utilizing movement therapy. The primary objective is to facilitate a return to function and community activities.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Implement therapy interventions to improve patient mobility and functional independence.
• Collaborate with other rehabilitation professionals to optimize patient care.
• Educate patients and their families on effective treatment strategies and home programs.
• Document patient progress and adjust treatment plans as necessary.

Required Experience / Certifications / Licensure

• Current unencumbered Oregon Physical Therapy license required.
• Healthcare Provider Level BLS certification required.
• Must be able to travel to multiple points of care.
