Job Description

The Hospice teams provide compassionate support to patients and their families through the final stages of illness at a 12-bed inpatient facility. Services include skilled nursing care, pain management, social services, counseling, spiritual support, and bereavement support.

Responsibilities

• Provide skilled nursing care and symptom management to hospice patients.
• Administer medications and manage pain control strategies.
• Collaborate with multidisciplinary teams to develop and implement care plans.
• Support patients' families with counseling and emotional support services.
• Assist with bereavement services for families post-loss.

Required Experience / Certifications / Licensure

• Current unencumbered Oregon RN Licensure required upon submission.
• Healthcare Provider Level BLS required.
• BSN and/or previous hospice or home health experience preferred.
• Broad-based nursing experience to ensure specialized skills for hospice care preferred.
