Schedule

Day shift, 5x8 hours (08:00 - 16:30). Please note that this facility cannot accommodate 4x10 or 3x12 schedules.

Job Description

The Home Health Physical Therapist will be responsible for providing skilled therapy services to patients in their homes. The role involves working independently to manage a caseload with a patient ratio of 1:1, visiting up to 6 patients per day and meeting a productivity standard of 30 points.

Responsibilities

• Conduct patient evaluations and implement treatment plans based on individual needs.
• Utilize Oasis for patient assessments and documentation.
• Educate patients and their families on home exercise programs and rehabilitation processes.
• Collaborate with interdisciplinary teams to optimize patient care.
• Document patient progress and treatment outcomes accurately.

Required Experience / Certifications / Licensure

• A minimum of 1 year of skilled home health experience is required.
• Current BCLS certification is mandatory.
• Oasis experience is required.
• Must have reliable transportation as occasional travel may be needed within a 50+ mile radius.
• COVID-19 vaccination is required, including boosters.
