Job Description

We are seeking a dedicated Home Health Physical Therapist to join our team in Medford, OR and the surrounding areas. This position involves providing therapy services to patients in their homes, ensuring optimal recovery and rehabilitation outcomes. The ideal candidate will have experience in skilled home health and be committed to delivering high-quality patient care.

Responsibilities

• Conduct evaluations and develop treatment plans tailored to individual patient needs.
• Implement therapeutic exercise programs and modalities to enhance recovery.
• Educate patients and families on home exercise programs and techniques for safe and effective recovery.
• Document patient progress through the Oasis system and ensure compliance with standards.
• Coordinate with interdisciplinary teams to facilitate comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum 1 year of skilled home health experience required.
• BCLS certification is required.
• Experience with Oasis documentation is necessary.
• Reliable transportation is mandatory for travel between patient homes.
