Job Description

The Home Health Physical Therapist will provide care to patients in their homes, focusing on rehabilitation and mobility improvement. This role involves traveling to patient locations and delivering personalized care based on individual treatment plans.

Responsibilities

• Conduct evaluations and develop treatment plans tailored to individual patient needs.
• Administer therapy interventions and monitor patient progress.
• Educate patients and their families on exercises and treatment processes.
• Complete necessary documentation using OASIS and EPIC charting systems.
• Collaborate with other healthcare professionals to provide comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Current Physical Therapist license for the state of Oregon.
• Basic Life Support (BCLS) certification required.
• Experience with OASIS documentation is required.
• Must have reliable transportation.
